Remove _token support altogether` #12986
Description
Reading through the code, it looks like a couple of places (and npm-registry-client) still handle the "extremely legacy" _token login token (known as couchToken in n-r-c).
As part of a breaking change, we could just take this out and just have a little less to worry about.
